Belper PCN Social Prescribing Leaflet

District Nurses – The District Nursing Team is based at the Practice and provide a service to patients who are housebound. They are in close touch daily with the Practice clinical staff and with the hospital doctors.

ACPs – Are Advanced Clinical Practitioners who work predominantly with the elderly patient group at the Surgery. 

The nurses offer consultations for a wide range of problems including:

• blood pressure monitoring
• cervical smears
• injections
• general health care advice
• Childhood Vaccinations

We are keen to provide preventative healthcare and screening facilities for our patients.
